Eligibility

ICS services are offered to adults residing in ICS settings. To find a list of authorized ICS settings, refer to CBSM – Resource: Integrated Community Supports (ICS) settings. If the provider does not own, operate, or lease the living units or has any direct or indirect financial interest in the property where services are provided, see CBSM – Individualized Home Supports as an alternative.

As of January 1, 2021, ICS services are available to adults under the following waivers:

• Brain Injury (BI)

• Community Access for Disability Inclusion (CADI)

As of January 1, 2023, or upon federal approval, ICS services are also extended to adults under these waivers:

• Community Alternative Care (CAC)

• Developmental Disabilities (DD)

Note: Include a document referral form for filling and faxing. Add links to the policy, procedures, and employee training materials. Provide apartment photos for each ICS unit, along with location details and availability.
